how does an SMG knob install? are they internally threaded?

No threads. The factory knob just pulls straight off and yes it takes a fair bit of force. This one uses a set screw after pushing on I believe, M spec can comment if I'm wrong concerning the Storm Motorwerks knob.

Correct has a set screw and pushes on...

didn't see a set screw in the pics, so i was curious. bmw makes things difficult with the non-threaded shifter rod. i'm trying to figure out to how to attach a certain shift knob now and looking for set screw inserts or something similar.

They install by set screws, although they slide firmly on. No modification needed to install!
